- Checklist for first appointment:
- ** Insurance Card (If your claim is to be paid by workers compensation,
- we will require the following information on the first visit: Date of
- Injury, Claim Number and/or Case Number, Telephone of Case
- Worker, and Fax Number of Case Worker)
- ** Photo ID
- ** Records from all physicians who have prescribed narcotics in the past
- year
- ** Records from all physicians in the past year who have treated
- your pain with injections or medications
- ** MRIs and CT films or CD of the films from the past 2 years
- ** A list of current medications
** A list of current allergies
** Without prior records we will not prescribe any medications
on the first visit. Please review our narcotic policy.
Please Arrive 15 minutes Prior to the First Scheduled Appointment (except
8:00 AM appointments) in Order to Complete Our Paperwork and Sign Our
Consent/HIPAA Forms. See our maps for directions.
Be Prepared to Pay the Copay at the Time of the Visit
If You are Private Pay without Insurance, the Full Amount is To Be
Paid at the Time of the Appointment
